A. Infectious and Genetic Transmissible Conditions
Malaria, Hepatitis viruses, Schistosomiasis, other Neglected Tropical Diseases (NTDs) including leishmaniasis, genetic conditions such as sickle cell disease and thalassemia.
B. Non-Communicable Diseases (NCDs)
Hypertension, Diabetes Mellitus, Obesity, Mental health disorders and Chronic respiratory conditions, including asthma.

As lifestyles evolve and health challenges shift, our efforts in health system strengthening and research address both endemic and emerging conditions through the following comprehensive core interventions.
• Risk assessment and screening for early identification of conditions.
• Linkage to appropriate treatment and care, including referral systems.
• Development of simplified training and capacity-building tools for paediatric and adolescent care.
• Support for immunization campaigns targeting herpes viruses, hepatitis, and routine childhood and maternal vaccines.
• Mass drug administration for prophylaxis of diseases such as malaria and schistosomiasis.
• Capacity building of frontline health workers to improve detection, management, and referral.
• Health system strengthening, including laboratory capacity, supply chain management, and effective use of routine health data.
• Monitoring and evaluation of programs targeting infectious, genetic, and non-communicable diseases to ensure impact and continuous improvement

Enhancing Care: Integrating Non-Communicable Disease screening and management into HIV care in Karamoja Region.

TRANSFORMING LIVES AND BUILDING RESILIENCE FOR PEOPLE LIVING WITH HIV

Background:
Karamoja region faced significant gaps in integration of Non-Communicable Diseases (NCDs) management into HIV care. Capacity gaps and challenges in documentation affected routine screening for NCDs resulting in only 8% of clients screened for Hypertension and 0% for both Diabetes Mellitus and Mental Health disorders among People Living with HIV in FY24 QTR1.
Intervention: A collaborative strategy was implemented including:
• Technical support and mentorship from regional health partners to empower healthcare workers with the skills for NCD screening, diagnosis and management.
• A user-friendly, one-page screening tool for Diabetes Mellitus and Mental Health was created to facilitate easy administration and documentation at health facility and community.
• Client appointments for ART delivery were restructured to allow for clinician assessments and follow-up for those diagnosed with NCDs.

Results: The initiative led to remarkable improvement in NCD screening among PLHIV over the four quarters. All clients diagnosed with NCDs were successfully linked to care and monitored consistently for stability.

Conclusion: Integrating NCD management into routine HIV care through mentorship, innovative documentation, and community engagement improved NCD screening and management.
Lessons Learned:
• Partnerships in capacity building enhance skills and knowledge of healthcare workers.
• Simple, user-friendly tools streamline screening processes and enhance documentation, leading to better tracking and patient management.
• Consistent follow-up for clients diagnosed with NCDs supports long-term health management.
